Reablement – the model for the future

Within our work at Essex we have been involved in the transformation and development of the former home care service into a new style ‘Reablement’ service combining the work of domiciliary workers with the skills of Occupational Therapists to deliver a new model and ethos of support particularly for people discharged from hospital. As a non chargable service a six week intensive package of support is provided with the emphasis on achieving independence for the service user. During the first 12 months the model has produced excellent outcomes underlining the vision of supporting people to find their own level of independence and support – this is very much the model for the future and directly links with the Personalisation Agenda now underway across Local Authority and County Councils.
